Before Your Visit
Do you work with personal injury attorneys?
We have experience treating patients whose injuries are associated with auto accidents and personal injury matters, and we maintain organized clinical documentation. With appropriate patient authorization, we can communicate with an attorney's office and respond to records requests.
Can my attorney refer me to your clinic?
Yes. Attorneys and case managers can submit a referral through our For Attorneys page, and our intake team will contact the patient to schedule an evaluation. Patients are always free to choose their own healthcare provider.
What should I bring to my first appointment?
Please bring a photo ID, your insurance information if applicable, a list of current medications, any imaging or records already completed, and your attorney's contact information if you are represented.
