After Accident Docs is an informational website that connects visitors with CMS Pain & Rehab. The contact form sends your name, email address, optional phone number and message to the clinic’s shared patient inquiry system, along with the website source and basic information used to prevent spam. The clinic uses this information to respond to your request. This website does not offer a separate public account or create a separate patient portal. Website hosting providers may process routine technical information such as IP addresses and request logs to deliver and secure the site.
This website does not use advertising trackers. Other websites and third-party tools linked from this site have their own data practices.
Scheduling and external services
The appointment page embeds the clinic’s Tebra scheduling service. Using that service sends information directly to the scheduling provider according to its own policies. You may also open it in a separate tab. The contact form and scheduling tool serve different purposes: sending a message does not confirm an appointment. The website does not receive a copy of the form fields you enter in that embedded scheduler.
